Appointments
If you are interested in becoming a new patient please call the office at (404) 448-3394. When you call provide all of your contact information and the reason you are seeking a consultation. I will contact you for an intake and assist you with scheduling the appointment.
Please note that if you have been a patient in the past and have not been in for an appointment for longer than one year, you will be considered a new patient.
COVID-19 / Illness Note
For your safety and the safety of our community we ask that,
If you or your child have any symptoms suggestive of COVID-19 (fever, headache of an unknown origin, cough that is not allergy related, loss of taste or smell) reschedule your appointment. There will be no charge for a late cancellation under these circumstances.
For non-COVID upper respiratory symptoms, we require that you wear a mask for the duration of the visit.
Video Appointments
If you would like to schedule a video appointment
Go to the “forms” tab. Fill out and sign the “Consent for Telemedicine” form.
Email the completed form to diane.seale@efmateo.com
If you already have an appointment scheduled it will be converted into a video appointment
If you don’t already have an appointment scheduled call the office at (404) 448-3394 and we will assist you with scheduling the appointment.
The form contains the instructions for logging onto the video platform.

Encrypted Zoom Meetings
In our continuous effort to keep things easy, we have moved to zoom meetings now that their security has improved. We use the highest levels of end-to-end encryption in our meetings to ensure our appointments stay strictly confidential.
Check your email at the time of your appointment and you’ll find the link to the meeting room also found below.
If you already have Zoom installed and prefer to join the appointment manually, the Meeting ID is 9114559184.
When you click the Launch button, or use the meeting ID, you’ll be asked for the password below.

You will be asked to allow permissions for the meeting, please allow everything requested.

To install or update the latest version of Zoom please click here.
When you click Launch Telemedicine Appointment, you MUST click Open Zoom Meetings and optionally check the box to always allow. This is what triggers Zoom to launch the meeting.
To protect your privacy we use the highest standards of security which requires the latest version of Zoom to be installed. If you have an older version of Zoom installed, the app will request an update automatically. Please allow the app to update and then you will in to the meeting as soon as it has finished.
